Say It Forward accepts referrals from GPs, specialists, case managers, support coordinators, and self-referrals. No complicated paperwork — just get in touch.
How to make a referral
Referrals can be made by anyone — the person themselves, a family member, carer, GP, or other health professional. There is no formal referral form required to get started.
Get in touch
Call or email to discuss your situation. We'll talk through your needs, funding type, and availability to find a time that works for our initial meeting.
Initial assessment
We'll complete an initial assessment in your home or community setting to understand your communication and mealtime needs.
Goal setting & therapy plan
Together we'll set individualised goals and develop a therapy plan that fits your life, your schedule, and your funding.
Ongoing support
This may be regular sessions, progress reviews, and/or communication with your care team to make sure therapy is working for you.
Funding options we accept
Say It Forward works with a range of funding types. Not sure which applies to you? Get in touch and we'll help you work it out.
NDIS
We support NDIS participants under the Capacity Building — Improved Daily Living. We work with both plan-managed and self-managed participants.
Reach out directly or contact your support coordinator to get started.
Aged Care
We work with clients funded through Home Care Packages (HCP) at all levels, and the Commonwealth Home Support Programme (CHSP).
Contact your home care provider or My Aged Care coordinator to arrange a referral.
Private
Private clients are welcome. We offer transparent, competitive fees. Please contact us for current fee information and to discuss your needs.
No referral needed — contact us directly to book.
NIISQ
We accept referrals through the National Injury Insurance Scheme Queensland (NIISQ) for people with serious injuries sustained in motor vehicle accidents.
A referral from your NIISQ case manager is required.
What to expect
In-home or community visits
Sessions take place where you are — at home, in an aged care facility, in a clinic or community space.
Telehealth available
Can't do in-person? Telehealth sessions are available for suitable clients across Australia
Flexible scheduling
Sessions available Wednesdays and Thursdays, 8am–4pm. We'll work with your schedule to find a time that suits.
Family and carer involvement
We actively involve family members and carers in therapy — their support makes a real difference to outcomes.
